Morphological Variations in the Case of Erythema Nodosum Leprosum

Leprosy is an ancient disease that has survived into modern times despite global efforts to eradicate it. An 82-year-old female presented with multiple erythematous plaques, papules, and nodules all over the body for the past 20 days, along with diminished touch sensation in the bilateral dorsum of the foot. All biochemical parameters were within the acceptable range. Rheumatoid factor, Antinuclear antibodies, Enzyme- linked immunosorbent assay for detecting Human immunodeficiency viruses infection and Venereal disease research laboratory test results were negative. Modified Ziehl-Neelsen staining (using 5% sulphuric acid) was positive for acid-fast debris within the macrophages. Acid- fast bacilli can be found both within and outside of macrophages andhistiocytes in leprosy lesions. In the early stages of infection, bacilli are often seen intracellularly, residing within the cytoplasm of host
cells which was the morphological variation seen in the present case.
