Exosomes have emerged as promising nanoscale therapeutic platforms with significant potential in precision medicine, targeted drug delivery, regenerative medicine, and disease management. These naturally occurring extracellular vesicles, secreted by various cell types, contain biologically active molecules including proteins, lipids, messenger RNA, microRNA, and other regulatory components that facilitate intercellular communication. Due to their unique biological properties, including low immunogenicity, biocompatibility, and ability to transport functional molecules between cells, exosomes have gained considerable attention as advanced tools for biomedical applications.
Recent advances in molecular biology and nanomedicine have demonstrated that exosomes can serve as both therapeutic agents and delivery vehicles for improving treatment outcomes. Their ability to carry genetic materials and bioactive molecules provides opportunities for developing targeted therapeutic strategies against complex diseases, including cancer, cardiovascular disorders, neurodegenerative conditions, and inflammatory diseases. Furthermore, engineered exosomes are being investigated for enhancing drug stability, improving tissue-specific delivery, and reducing systemic toxicity associated with conventional therapeutic approaches.
This review discusses the biological characteristics, formation mechanisms, and molecular functions of exosomes, highlighting their role in maintaining cellular communication and regulating physiological processes. The article further examines the emerging applications of exosome-based therapeutics in precision medicine, including their potential use in targeted drug delivery, tissue regeneration, immunotherapy, and disease-specific treatment strategies. Current challenges related to large-scale production, isolation methods, quality control, safety assessment, and clinical translation are also discussed.
Although several technological and regulatory challenges remain, continuous advancements in biotechnology, molecular engineering, and nanomedicine are accelerating the development of exosome-based therapeutic approaches. The integration of exosome technology with personalized medicine strategies may provide innovative solutions for improving therapeutic precision, enhancing treatment effectiveness, and addressing unmet clinical needs in modern healthcare.
